fa بررسی شاخص‌های مختلف چاقی و شرایط پیش التهابی موجود در مایع فولیکولی زنان مبتلا به سندرم تخمدان پلی‌کیستیک Evaluations of different indicators of obesity and pro-inflammatory conditions in the follicular fluid of women with polycystic ovary syndrome زیست شناسی Biology موردي- شاهدي case-control سابقه و هدف: سندرم تخمدان پلیکیستیک (PCOS) نوعی اختلال غدد درونریز است که بهعنوان بیماری پیشالتهابی درنظر گرفته    میشود. چاقی نیز یک فنوتیپ پیشالتهابی شایع در بیماران PCOS است که ممکن است در شروع یا تشدید التهاب در بیماران PCOS نقش داشته باشد. در این مطالعه، نقش شاخصهای مختلف چاقی در افزایش التهاب در مایع فولیکولی بیماران PCOS بررسی شده است.   روش بررسی: 40 بیمار زیر 35 سال به دو گروه PCOS و non-PCOS تقسیم شدند. شاخصهای مختلف چاقی (توده بدنی، دور گردن، دور بازو و دور کمر به لگن) در هر بیمار اندازهگیری شد. نمونههای مایع فولیکولی از هر بیمار جمعآوری شد. غلظت فاکتور نکروز دهندهی تومور- (TNF-α) α در نمونه‌های مایع فولیکولی بیماران، به‌طور کمّی با استفاده از روش الایزا ارزیابی شد. تعداد کمپلکس‌های کومولوس-اووسیت (COC) بازیابی شده و کیفیت آنها امتیازدهی شد.  یافتهها: در گروه PCOS میزان شاخصهای مختلف چاقی به‌طور معنی‌داری نسبت گروه non-PCOS افزایش داشت (001/0P ≤). غلظت TNF-α در گروه PCOS بهطور معنی‌داری بیشتر از گروه دیگر بود (001/0P <). تفاوت معنیداری در تعداد COC بازیابی شده به ازای هر بیمار و کیفیت تخمک بین گروهها وجود نداشت (05/0 P>). نتیجهگیری: با توجه به نقش مهم التهاب در ایجاد بیماری PCOS، مدیریت چاقی در بیماران PCOS ممکن است به کاهش التهاب کمک کند و بهطور بالقوه می‌تواند به طراحی رویکردهای پزشکی منحصر   به فرد کمک کند.   Background: Polycystic ovary syndrome (PCOS) is an endocrine disorder and is considered a pro-inflammatory disease. Obesity is also a common pro-inflammatory phenotype in PCOS patients, which may play a role in the initiation or exacerbation of inflammation in PCOS patients. In this study, the role of various obesity indicators in increasing inflammation in the follicular fluid of PCOS patients has been investigated. Materials and methods: 40 patients under 35 years were divided into two groups:  PCOS and non-PCOS. Different obesity indices (body mass, neck circumference, arm circumference, and waist to hip circumference) were measured in each patient. Follicular fluid samples were collected from each patient. The concentration of tumor necrosis factor- α (TNF-α) in follicular fluid samples of patients was quantitatively evaluated using the ELISA method. The number of cumulus-oocyte complexes (COC) was recovered and their quality was scored. Results: In the PCOS group, various obesity indices were significantly increased compared to the non-PCOS group (P ≤0.001). The concentration of TNF-α in the PCOS group was significantly higher than the other group (P<0.001). There were no significant differences in the number of retrieved COC per patient and the quality of oocytes between the groups (P>0.05). Conclusion: Considering the important role of inflammation in the development of PCOS disease, management of obesity in PCOS patients may help to reduce inflammation and can potentially help to design personalized therapeutic approaches.   سندرم تخمدان پلی کیستیک, چاقی, التهاب, فاکتور نکروز دهنده تومور – α.
