fa بررسی شیوع منابع آلودگی سویه های انتروهموراژیک اشریشیاکلی و میزان مقاومت آن ها در مقابل آنتی بیوتیک ها در بین کودکان زیر 5 سال شهرستان مرودشت اپيدميولوژي Epidemiology <span style="font-weight: bold">سابقه و هدف</span>: سویه های انتروهموراژیک اشریشیاکلی یکی از مهم ترین پاتوژن های روده ای ایجاد کننده بیماری های منتقل شونده به وسیله مواد غذایی در انسان محسوب می شوند. هدف از این پژوهش، ارزیابی شیوع اسهال ناشی از این باکتری و بررسی منابع آلودگی و میزان مقاومت آنتی بیوتیکی سویه های جداسازی شده از کودکان زیر 5 سال در شهرستان مرودشت بود.<br><span style="font-weight: bold">روش بررسی</span>: در این پژوهش توصیفی نمونه مدفوع کودکان زیر 5 سال شهرستان مرودشت طی یک سال (1386-1385) جمع آوری و پس از غنی سازی در محیط کشت های اختصاصی و ارزیابی تخمیر سوربیتول و فعالیت بتاگلوکورونیدازی تست(MUG)، با کمک آنتی سرم اختصاصی، سویه های انتروهموراژیک O157:H7 تعیین هویت گردید. ژن های بیماری زای وروتوکسین، اینتیمین و همولیزین با استفاده از روش Multiplex PCR و مقاومت آنتی بیوتیکی سویه ها با روش دیسک دیفیوژن مورد ارزیابی قرار گرفت. آلودگی احتمالی تمامی کودکان با آب، مواد غذایی و تماس با حیوانات نیز بررسی شد.<br><span style="font-weight: bold">یافته ها</span>: از 615 نمونه مورد بررسی، 89 کلنی سوربیتول منفی جداسازی گردید که 7 سویه O157 14/1) درصد) با آنتی سرم اختصاصی تایید شد. تنها در یکی از سویه های جدا شده، ژن های stx1 و eaeA 16/0) درصد) مشاهده گردید. تمامی باکتری های جداسازی شده به آنتی بیوتیک های پنی سیلین، آمپی سیلین و اریترومایسین مقاومت داشتند. بین جداسازی سویه های انتروهموراژیک اشریشیاکلی و فصول مختلف سال ارتباط معنی داری وجود نداشت.<br><span style="font-weight: bold">نتیجه گیری</span>: پایش بیمارستانی به منظور برآورد شیوع سویه های انتروهموراژیک در مراکز درمانی در تمامی کودکان زیر 5 سال مبتلا به گاستروانتریت حاد و ارزیابی اتیولوژیکی و ژنوتایپینگ باکتری در سایر مناطق کشور پیشنهاد می گردد. <span style="font-weight: bold">Background</span>: Enterohemorrhagic Escherichia coli (EHEC) strains are one of the most important enteric pathogens which cause of the food-borne disease in humans. The purpose of this study was to survey the prevalence of diarrhea arising from this bacteria and infection source and antibiotic resistance of isolated strains in children under 5 years old in Marvdasht. <br><span style="font-weight: bold">Materials and methods</span>: In this descriptive study, Stool samples of under 5 years old children in Marvdasht were collected. After enrichment in specific culture media and evaluation sorbitol fermentation and their β-glucoronidase activity (MUG test) with specific antisera, the isolation of EHEC O157:H7 strain was confirmed. Then, virulence genes verotoxin, intimin and hemolysin with multiplex PCR and antibiotic resistance strains with disk diffusion method were tested. Also, getting infection with water, food and animal contact was evaluated. <br><span style="font-weight: bold">Results</span>: Out of 615 cases, 89 sorbitol negative colonies isolated which 7 strains O157 (1.14%) confirmed with antisera. Of virulence genes, only 1 of the isolated strains (0.16%) had stx1 and eaeA genes. All isolated bacteria had resistance to penicillin, ampicillin and erythromycin.
