fa بررسی لوپوس القا شده دارویی در افراد بالای ۲۰ سال مبتلا به بیماری‌های التهابی روماتولوژیک مصرف کننده anti-TNFα مراجعه کننده به بیمارستان بوعلی تهران در طی سال‌های ۱۴۰۲-۱۳۹۹ Investigation of clinical and laboratory symptoms of drug-induced lupus in patients over 20 years old with inflammatory rheumatic diseases using anti-TNFα, referring to BooAli Hospital in Tehran during the years 2020-2023 روماتولوژي Romatology تحليلي/مقطعي/توصيفي Survey/Cross Sectional/Descriptive <v:line from="3.6pt,2.8pt" id="_x0000_s1026" strokeweight="1.5pt" style="position:absolute; left:0; text-align:left; z-index:251656704" to="480.6pt,2.8pt"> <w:wrap anchorx="page"> </w:wrap></v:line><img alt="" src="file:///C:/Users/afsane/AppData/Local/Temp/msohtmlclip1/01/clip_image001.gif" style="width:639px; height:2px" >چکیده سابقه و هدف: لوپوس اریتماتوز سیستمیک (SLE) نوعی بیماری خودایمنی مزمن است که با تولید اتونتی‌بادی‌ها علیه آنتی ژ‌ن‌های هسته‌ای مشخص می‌شود. نوعی از این بیماری، لوپوس القا شده دارویی (DILE)، با مصرف مداوم برخی داروها، از جمله داروهای anti-TNFa رخ می‌دهد. این مطالعه با هدف بررسی تظاهرات بالینی و آزمایشگاهی لوپوس القا شده دارویی در افراد بالای ۲۰ سال مبتلا به بیماری‌های التهابی روماتولوژیک تحت درمان با Anti-TNFα در بیمارستان بوعلی تهران انجام شد. روش بررسی: این مطالعه مقطعی و توصیفی- تحلیلی روی ۴۶ بیمار (۱۳ مرد و۳۳زن) با میانگین سنی ۵۵ سال انجام شد. بیماران مبتلا به آرتریت روماتویید، پسوریازیس و اسپوندیلوآرتروپاتی بودند که حداقل یک سال از شروع بیماری‌شان گذشته بود و طبق برنامه درمانی نیاز به داروهای Anti-TNFα داشته اند. ارزیابی‌های بالینی و آزمایشگاهی قبل و شش ماه پس از شروع درمان با اتانرسپت یا ادالیموماب انجام شد.  یافتهها: پس از شش ماه درمان با Anti-TNFα، تظاهرات بالینی مرتبط با DILE مشاهده نشد. درصد بیماران با ANA  مثبت به‌طور     معنیداری از ۳۹/۱۷ به ۶۰/۳۲ افزایش یافت. سایر پارامترهای آزمایشگاهی تغییر معنیداری نداشتند. کاهش معنیداری در سطح RF و Anti-CCP مشاهده شد، نشان‌دهنده سرکوب فرآیند التهابی است. نتیجهگیری: از آنجایی که این دسته از داروها نقش مهمی در درمان بیماریهای روماتولوژیک دارند، نتایج مطالعه بیانگر تاثیرگذاری درمان در کاهش علائم بیماری التهابی روماتولوژیک و تغییر در شاخصهای مرتبط با بیماری فرد است و ادامه درمان با این داروها تا پیری در افزایش بروز لوپوس ناشی از دارو ندارد.   Background: Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by the production of autoantibodies against nuclear antigens. Drug-induced lupus erythematosus (DILE) is a type of SLE that occurs as a result of the prolonged use of certain medications, including anti-tumor necrosis factor alpha (anti-TNFα) agents. This study aimed to investigate the clinical and laboratory manifestations of drug-induced lupus in individuals over 20 years of age with inflammatory rheumatologic diseases treated with anti-TNFα at BooAli Hospital in Tehran. Materials and methods: This cross-sectional, descriptive-analytical study was conducted on 46 patients (13 men and 33 women) with a mean age of 55 years. Patients had rheumatoid arthritis, psoriasis, or spondyloarthropathy and had been suffering from their disease for at least one year. Clinical and laboratory assessments were performed before and six months after the initiation of treatment with etanercept or adalimumab. Results: After six months of treatment with anti-TNFα, no clinical manifestations related to DILE were observed. The frequency of patients with positive ANA significantly increased from 17.39% to 32.60%. Other laboratory parameters did not show significant changes. A significant decrease in RF and Anti-CCP levels was observed, indicating suppression of the inflammatory process. Conclusion: Treatment with anti-TNFα in patients with inflammatory rheumatic diseases leads to an increase in positive ANA, but this increase does not lead to the clinical manifestations of DILE. The decrease in RF and Anti-CCP levels indicates the effectiveness of the treatment in controlling disease activity.   لوپوس اریتماتوز سیستمیک, لوپوس القا شده دارویی, اتانرسپت, ادالیموماب, بیماری‌های التهابی روماتولویک.
