fa بررسی رابطه ترومبوسایتوپنی در بیماران با عفونت هلیکوباکترپیلوری مثبت به روش تست اوره‌آز سریع بافتی (RUT) مراجعه‌کننده به بیمارستان‌های دانشگاه علوم پزشکی آزاد اسلامی تهران در سال‌های 1404-1402 Study of the frequency of thrombocytopenia in patients with positive Helicobacter pylori infection by rapid tissue urease test (RUT) referred to hospitals of Tehran Islamic Azad University of Medical Sciences in 2023-5025 گوارش Gastrology تحليلي/مقطعي/توصيفي Survey/Cross Sectional/Descriptive <v:line from="3.6pt,2.8pt" id="_x0000_s1026" strokeweight="1.5pt" style="position:absolute; left:0; text-align:left; z-index:251658752" to="480.6pt,2.8pt"> <w:wrap anchorx="page"> </w:wrap></v:line> چکیده سابقه و هدف: عفونت هلیکوباکترپیلوری یکی از بیماری‌های شایع دستگاه گوارش است که می‌تواند اثرات سیستمیک از جمله ناهنجاری‌های خونی مانند ترومبوسایتوپنی ایجاد کند. این مطالعه با هدف بررسی فراوانی ترومبوسایتوپنی و عوامل مرتبط با آن در بیماران مبتلا به عفونت هلیکوباکترپیلوری انجام شد. روش بررسی: این مطالعه مقطعی توصیفی روی ۱۵۴ بیمار مراجعه‌کننده به بخش اسکوپی بیمارستان‌های تابع دانشگاه آزاد اسلامی واحد پزشکی تهران در بازه زمانی سال‌های ۱۴۰2-۱۴۰4 انجام شد. بیماران از بخش‌های آندوسکوپی بیمارستان‌های تابعه دانشگاه علوم پزشکی آزاد اسلامی تهران انتخاب شدند. داده‌ها شامل مشخصات دموگرافیک، علائم بالینی و نتایج آزمایشات خون (CBC و شمارش پلاکت) جمع‌آوری شد. تحلیل آماری با استفاده از نرم‌افزار SPSS  نسخه ۲۶ و سطح معنیداری 05/0 انجام شد.  یافتهها: از بین ۱۵۴ بیمار، ۸۱ نفر (6/52 درصد) زن و ۷۳ نفر (4/47 درصد) مرد بودند و میانگین سن آن‌ها 95/14 ± 95/50 سال بود. ترومبوسایتوپنی در ۱۰ بیمار (5/6 درصد) مشاهده شد. ارتباط معنیداری بین ترومبوسایتوپنی و جنسیت، وضعیت دیابت، درد شکم، تهوع و استفراغ  یا زخم معده یافت نشد (05/0P >)، اما ارتباط معنیداری بین نتایج مثبت تست RUT و ترومبوسایتوپنی مشاهده شد (001/0˂P). نتیجهگیری: این مطالعه ارتباط بالقوه‌ای بین عفونت هلیکوباکترپیلوری و ترومبوسایتوپنی را نشان می‌دهد. این ارتباط نشان دهنده تاثیر عفونت فعال بر شمارش پلاکت است. لذا تحقیقات بیشتر برای روشن شدن مکانیسم‌های مرتبط با این رابطه توصیه می‌شود Background: Helicobacter pylori infection is a common gastrointestinal condition with potential systemic effects, including hematological abnormalities such as thrombocytopenia. This study aimed to assess the prevalence of thrombocytopenia and its associated factors in patients with confirmed Helicobacter pylori infection. Materials and methods: This cross-sectional descriptive study included 154 patients in scopy wards of Islamic Azad University of Medical Science Hospitals from 2023 to 2025. Patients were recruited from the endoscopy units of hospitals affiliated with Tehran Islamic Azad University of Medical Sciences. Data were collected on demographic characteristics, clinical symptoms, and laboratory findings, including complete blood count (CBC) and platelet counts. Statistical analysis was performed using SPSS version 26, with a significance level of P < 0.05.   Results: Among the 154 patients with a mean age of 50.95 ± 14.95 years, 81 (52.6%) and 73 (47.4%) were female and male, respectively. Thrombocytopenia was identified in 10 patients (6.5%). No significant associations were observed between thrombocytopenia and gender, diabetes status, abdominal pain, nausea/vomiting, or gastric ulcer (P> 0.05). However, a significant association was found between positive RUT results and thrombocytopenia (P< 0.001). Conclusion: The study highlights a potential link between Helicobacter pylori infection and thrombocytopenia, as indicated by the significant association with positive RUT results. While common symptoms and comorbidities were not predictive of thrombocytopenia, active infection may influence platelet counts.
