بررسی اثر عصاره انار در حفاظت از غضروف مفصل زانو در مدل تجربی استئوآرتریت
Chondro-protective effects of pomegranate on experimental osteoarthritis

<p><strong>سابقه و هدف:</strong> در این پژوهش جهت بررسی اثر عصاره انار (PJ) بر استئوآرتریت (OA) از تزریق داخل مفصلی منویدواستات (MIA) در مفصل تیبوفمورال موش سوری استفاده شده است. آب انار به علت دارا بودن خواص آنتی اکسیدان و همچنین اثرات پیشگیرنده در بیماریهای قلبی، از مقبولیت زیادی برخوردار است، اما تاکنون هیچ مطالعه هیستوپاتولوژیکی در حیوانات جهت بررسی اثرات آن در جلوگیری از تخریب غضروف مفصلی و یا ترمیم آنها انجام نشده است.</p><p><strong>روش بررسی:</strong> در این مطالعه تجربی، ابتدا تغییرات هیستوپاتولوژیک استخوان ساب کندرال و غضروف مفصل زانوی موش سوری در طی 1، 14 و 28 روز بعد از تزریق 0/1mgمنویدواستات (MIA) مورد بررسی قرار گرفت. سپس اثرات تجویز خوراکی عصاره انار در گروههای مختلف بررسی گردید. در گروه اول عصاره انار با دوز 4ml/kg ، در گروه دوم عصاره انار با دوز 10ml/kg و در گروه سوم عصاره انار با دوز 20ml/kg تجویز و تغییرات هیستوپاتولوژیک در مفاصل زانو دو هفته بعد مورد مطالعه میکروسکوپی قرار گرفت.</p><p><strong>یافته ها:</strong> مراحل اولیه استئوآرتریت از نظر پاتولوژیکی شامل مناطقی از دژنراسیون و نکروز کندروسیت هاست که گاهی اوقات تمامی ضخامت غضروف مفصل را درگیر می سازد. تغییرات استخوان ساب کندرال و پروتئوگلیکان شامل کلاپس ترابکول های استخوانی، فیبروز و نکروز و پرولیفراسیون سینوویوم می باشد. تجویز عصاره انار در گروه های مختلف موش سوری به صورت وابسته به دوز مانع از اثرات MIA در ایجاد OA می شود. این اثرات به ویژه در گروههایی که دوز بالاتری از عصاره انار را دریافت کردند، به طور موثری مشاهده گردید که از آسیب کندروسیت ها جلوگیری به عمل آورده و تغییرات کمتری در پروتئوگلیکان ایجاد شده است.</p><p><strong>نتیجه گیری:</strong> تزریق تک دوز داخل مفصلی MIA منجر به آسیب سریع و پیشرونده غضروف مفصلی شده که دقیقا استئوآرتریت در انسان را تقلید می نماید. در این پژوهش، اثر عصاره انار در بهبودی آسیبهای هیستوپاتولوژیک غضروف مفصل موش سوری مورد تائید قرار گرفت. </p>
<p><strong>Background:</strong> To study the effectiveness of pomegranate juice (PJ) on osteoarthritis (OA), we used mono-iodoacetate (MIA), an inhibitor of glycolysis, in tibiofemoral joint of mice that promotes loss of articular cartilage similar to that noted OA. Pomegranate juice (PJ) is increasing in popularity because of its high antioxidant content, already known to help prevent heart disease. However, no histopathological studies have undertaken in vivo to investigate whether PJ protect articular cartilage. </p><p><strong>Material and methods:</strong> We described the histopathology in the subchondral bone and cartilage of mice knee joint treated with a single intra-articular injection of MIA (0.1 mg) and sacrificed at 1, 14 and 28 days post injection. Then, the beneficial effect of oral PJ was studied in different groups group 1: administration of PJ (4ml/kg), group 2: administration of PJ (10ml/kg) and group 3: administration of PJ (20ml/kg). Histopathological changes in knee joints were studied after two weeks. </p><p><strong>Results:</strong> Histologically, the early OA was characterized by areas of chondrocytes degeneration/ necrosis sometimes involving the entire thickness of the articular cartilage in the tibial plateaus and femoral condyles. Changes to the subchondral bone and proteoglycane contents, were observed and also, there was focal fragmentation and collapse of bony trabeculae with fibrosis and necrosis. Synovial cell proliferation was noted. Interestingly, administration of PJ in different group of mice prevented the negative effects of iodoacetate, in a dose dependent manner. Chondrocyte damages were significantly prevented and proteoglycane were less affected, especially in group receiving high amount of PJ and no cell proliferation and inflammatory cell were detected in synovium.</p><p> <strong>Conclusion:</strong> Fast and progressive damage to articular cartilage is induced by single intra-articular injection of MIA, which mimic exactly human OA. In this study, the effectiveness of PJ in improvement of histopathological damages is emphasized and its chondroprotective effects in vivo are highlighted. </p>
