Study of aspergillosis risk factors in high risk patients

Background: Invasive aspergillosis (IA) is a major opportunistic infection in immunocompromised patients. Aspergillus is the main cause of mortality due to infection after transplantation and remains a major complication in hematologic malignancies. Despite considerable progresses in infection management, aspergillosis remains an important cause of morbidity and mortality, mainly after transplantation.
Materials and methods: During an 11-year period (April 1996 to March 2006), a retrospective study was achieved on 24 patients with aspergillosis in order to identify significant risk factors for invasive aspergillosis. Diagnosis was confirmed by demonstration of fungi in direct preparation and culture techniques. Results: Aspergillosis was more commonly isolated from respiratory tract (75%). Patients had at least one predisposing factor for aspergillosis. Among patients with solid organ transplantation, kidney transplant patients and chronic granulomatos disease were at the higher risk of developing invasive aspergillosis (IA). Conclusion: Major advances in management of invasive fungal infections (IFI) have resulted from our understanding of the risk factors for the development of IFI. However, much remains to be done to decrease mortality due to IFI in high risk patients. A high degree of awareness and efforts for an early diagnosis may improve poor prognosis. This study signifies that clinicians should be alerted of potential fungal etiology in cases of fever, which is unresponsive to conventional medical therapy.

Surveying the effect of 100 kD protein of shark cartilage on erythroleukemic K562 cell line and normal lymphocytes

Background: Shark cartilage is used in complementary medicine for the removal of tumor in cancer patients. However, scanty reports have addressed the effect of 100 KDa fraction on immunity system. The aim of the present study was to evaluate the effect of this fraction in human erythroleukemic K562 cell line and normal lymphocytes.
Materials and methods: Gel chromatography purification strategy was employed for purification of 100 KDa fraction from the shark cartilage. Erythroleukemic K562 cell line and normal lymphocytes were cultured in RPMI 1640 (Sigma), supplemented with 10% fetal calf serum, peniciline-streptomycin, L-glutamine and incubated at 37 ºC for 2 days. Normal mononuclear peripheral blood cells were collected by ficole. Cellular vital capacity was determined by MTT.
Results: During MTT, erythroleukemic K562 cell line revealed to have a meaningful suppression at 15g protein concentration when compared with controls (p<0.01). Conclusion: To our knowledge, it is the first study that demonstrates 100kD protein of shark cartilage to induce cell death in erythroleukemic K562 cell line in vitro.

Effect of seminal plasma selenium concentration on semen parameters

Background: Selenium is an essential trace nutrient for humans and animals. It is required for normal testicular development and spermatogenesis. In the present experimental study, correlations between seminal plasma glutathione peroxidase enzyme activity (as selenium status) and semen parameters are evaluated in 200 males.
Materials and methods: Semen analysis was performed according to World Health Organization guidelines. Totally, 200 males were subdivided into 5 groups as normospermic, oligospermic, asthenozospermic, azospermic and varicocele groups according to their spermograms. Plasma seminal glutathione peroxidase enzyme activity was determined by Kit (Randox, Germany). Results: The results showed that glutathione peroxidase enzyme activity is higher in normospermic than oligospermic, asthenozospermic, azospermic and varicocele groups. Meanwhile, there were an inverse significant correlations between glutathione peroxidase enzyme activity and plasma seminal fructose concentration, white blood cell, tail defects of sperm, coiled tail sperms and short tail sperms. On the other hand, the present data showed that there are positive correlations between vitality, sperm count, motility and normal morphology.
Conclusion: The present study indicated that measurement of glutathione peroxidase enzyme activity as selenium status could be a good marker for evaluation of male infertility.

Correlation between red blood cell membrane fatty acids and plasma lipids with clinical severity of retinitis pigmentosa

Background: Retinitis pigmentosa (RP) is an inherited retinal dystrophy caused by the loss of photoreceptors and characterized by retinal pigment deposits visible on fundus examination. Low red blood cell membrane fatty acid content e.g. docosahexaenoic acid (DHA) in retinitis pigmentosa may influence retinal function. The aim of this study was to evaluate red blood cell membrane fatty acid content and plasma lipid levels and determine their associated effects on the severity of disease. Materials and methods: In a comparative cross-sectional study, 35 males and 16 females with RP (mean age= 35.7 years) were evaluated. Red blood cell membrane fatty acid content (DHA) and plasma lipid levels were measured as well as visual acuity and field.
Results: Both visual acuity and visual field were significantly associated with red blood cell membrane fatty acid content (DHA) (P<0.05). However, the plasma lipid levels, except of total cholesterol, were not associated with visual acuity and visual field of RP patients (NS).
Conclusion: Current survey showed a significant association between severity of RP and fatty acid content (DHA).

Effects of formaldehyde on morphometric structure of testis in Balb/C mice

Background: Formaldehyde is a suffocating gas with a sharp smell which enters the body from different ways and induces tissue damages. With respect to its effects damages on testis and the on testis and spermatozoa, the present study was conducted to determine the effects of formaldehyde on morphometric structure of testis in Balb/C mice.
Materials and methods: In an experimental research, a total of 40 adult mice of the bread Balb/C were selected and assigned in experimental and control groups. For this study 37% formaldehyde was injected intraperitoneally with four doses in a 40-day period. After weighting the mice, the abdomen was opened and the testes were weighted and subjected to morphometric study.
Results: Results showed that there were not significant differences between the experimental group and controls based on testes weight and morphometric structure, however, body weight and the ratio of the weight of two testes to body weight (GSI) did differ significantly.
Conclusion: It was showed that despite a significant reduction in body weight following the formaldehyde injection, the weight of testes and their dimensions were not changed.

Effects of formaldehyde on morphometric structure of testis in Balb/C mice

Background: Children's physical growth is of utmost importance and their height and weight measurements are the most valuable factors in order to study and evaluate the quantity and quality of their growth. In Iran, like many other countries, NCHS standard charts are used, however, due to ethnic and geographical variations local charts should be prepared in order to compare children’s status of health and growth.
Materials and method: During the present cross sectional study, some primary schools were selected randomly and the height and weight of all 7-11 years old students (2224 students) were measured. Those with known disease affecting growth were excluded. Results: Our results indicated that the mean of height and weight of 7 years old girls had been 120.04cm and 21.23kg, respectively, however, these figures increased to 140.49cm and 31.88kg by the age of 11 years. These figures were 119.32cm and 21.5kg and 139.36cm and 31.98kg for 7 and 11 years old boys, respectively. Conclusion:Results revealed that the mean height and weight of children in Shahrood is lower than the international standards. This suggests that nursing education should be improved to provide a higher level of health services. Also parents, teachers and school health care providers need some further training regarding children nourishment, health, vaccination and disorders and disease prevention, as these factors have critical roles in children's physical growth.

Epidemiology and risk factors of 377 AIDS (HIV infected) patients

Background: Oral manifestations of HIV infection are common and include oral lesions and novel presentations of previously known opportunistic diseases. Some risk factors are significantly contributed to probability of developing opportunistic infection of oral candidiasis. We have performed the current survey to evaluate their role in Iranian patients with HIV infection.
Materials and methods: Totally, 377 documented HIV infected patients were included. Oral candidiasis was investigated with clinical observation, direct smear, and culture. Meanwhile, CD4 count was determined at the first visit and following antiretroviral therapy.
Results: The study population included 316 males and 60 females with a mean age of 36.5±8.7 years. Patients were diagnosed for a mean duration of 2.9 years. The most common route of infection transmission was shared needles (117 cases or 31%). In serologic tests, 11 cases (2.9%) were positive for HBsAg, 206 cases (71.5%) for HCV nucleic acid and 53 cases (14.1%) for tuberculosis infection. At the first visit 52 cases (13.9%) and following HAART 31 cases (8.2%) were revealed to have oral candidiasis. Mean CD4 count in patients with oral candidiasis was significantly less than other patients (193.8 vs. 349.7 cells, P=0.0001) however, the difference was not significant at follow up.
Conclusion: Prompt diagnosis and treatment of HIV infection may have a significant role in prognosis and clinical course of patients with HIV infection and could decrease the rate of opportunistic infections.

Surveying mental health status of nursing and non-nursing students of Shahroud Islamic Azad University

Background: When we speak of mental health, we consider a fresh and cheerful person who live and work with strong steps and friendly look. Activity, movement and freshness are signs of a healthy person and mental health is required for maintenance the social, job and educational activity. The present study was conducted to compare the mental health among female nursing and non-nursing students of Shahroud Islamic Azad University. Materials and methods: Totally, 180 nursing and 180 non-nursing students were selected randomly. They were asked to complete General Mental Health Questionnaire (GHQ-28). Then results were analyzed using descriptive and inferential statistics.
Results: Although there were no statistically significant differences between nursing and non-nursing students regarding their mental health and its criteria (physical disorders, anxiety, sleep disorders, social behavior, and depression), both groups showed a high psychasthenia status.
Conclusion: Results showed that university students are prone to losing their mental health due to their condition. However, depression, anxiety and other mental disorders may lead to decreased educational performance.

Association between computer use and feeling of students about their school and school exercises

Background: Spread of computer use and various ability of it can affect the life style of people, especially students. The present study was conducted to determine the patterns of computer use among students and their feeling about school and exercises.
Materials and methods: An analytic cross-sectional study was carried out on 1872 primary, middle and high school students by cluster sampling method in Damavand city. The number of hours that spend for computer in weekdays and weekends and feeling about school and exercises were answered by students in a standard questionnaire.
Results: Computer use by boys were significantly more than girls (P=0.0005). In weekdays, with increasing the educational stage, computer use was increased (p=0.03). Bad feeling about school and exercises was associated with more computer use in our subjects (p<0.01).
Conclusion: Pediatricians should carry out health education on the number of hours students should spend in front of the computer.

Endometrial cancer consisting of both adenosquamous and serous papillary cells with raised β-hCG: a case report

A 69-year old menopause woman presented with severe vaginal bleeding lasting for 70 days. Sonographist reported a 7mm mass in uterus, however, serologic studies revealed an elevated β-hCG level. Patient was subjected for total hysterectomy and bilateral salpingoophorectomy, during which endometrial cancer tissue consisting of both adenosquamous and serous papillary cells were noted. β-hCG level is rarely supposed to elevate in endometrial cancer, however, its association with endometrial cancer should be further discussed.
