مقایسه اثر اسانس شوید و مفنامیک اسید بر مدت درد بعد از زایمان:کارآزمایی بالینی تصادفی شده

چکیده:   (440 مشاهده)
سابقه و هدف: درد بعد از زایمان، از مشکلات دوران نفاس است. مهارکننده­های پروستاگلاندین مانند داروهای ضدالتهابی غیراستروئیدی معمولا برای کاهش درد استفاده می­شوند،  اما ممکن است باعث عوارض جانبی دستگاه گوارش شوند. در این مطالعه، اثر اسانس شوید و مفنامیک اسید بر مدت درد بعد از زایمان مقایسه شد.
روش بررسی: 108 زن چندزایی که در بیمارستان رازی اهواز به روش طبیعی زایمان کرده و درد بعد از زایمان به روش تصادفی ساده در دو گروه 54 نفری اسانس شوید (mg/kg5/1) و مفنامیک اسید (کپسول 250میلی گرم) قرار گرفتند. میزان درد 2 ساعت پس از زایمان با استفاده از مقیاس دیداری درد اندازه گیری شد. در بیمارانی که نمره درد 1/3 و بالاتر داشتند، در صورت ادامه یافتن درد هر 6 ساعت تا 4 بار، اسانس یا مفنامیک اسید تجویز شد. به دنبال مصرف اسانس شوید یا مفنامیک از بیمار خواسته می­شد زمان اثر دارو، زمان از بین رفتن کامل درد و شروع مجدد درد بعدی را اعلام کند. تحلیل داده­ها با نرم افزار SPSS نسخه22 و با استفاده از آزمون­های t، کای دو و اندازه­گیری مکرر انجام شد. 
یافته ­ها: مدت درد در طول زمان در هر گروه کاهش معنی­داری داشت (نوبت اول و چهارم اسانس شوید 21/3± 2/39 و 89/1±07/32 دقیقه و نوبت اول و چهارم مفنامیک اسید 09/4± 5/40 و 15/3± 97/31 دقیقه) (001/0>P). میانگین زمان اثر دارو بعد از مداخله نوبت دوم و سوم به ترتیب در گروه اسانس (74/2±89/20 و 05/2±24/19 دقیقه) تفاوت آماری معنی­داری با گروه مفنامیک (85/3±34/22 و 55/3±12/21 دقیقه) داشت (005/0>P).
نتیجه­گیری: شروع اثر اسانس شوید سریع­تر از مفنامیک است و می­توان از اسانس به دلیل نداشتن عارضه گوارشی استفاده کرد.
