Background: Helicobacter pylori infection is a common gastrointestinal condition with potential systemic effects, including hematological abnormalities such as thrombocytopenia. This study aimed to assess the prevalence of thrombocytopenia and its associated factors in patients with confirmed Helicobacter pylori infection. Materials and methods: This cross-sectional descriptive study included 154 patients in scopy wards of Islamic Azad University of Medical Science Hospitals from 2023 to 2025. Patients were recruited from the endoscopy units of hospitals affiliated with Tehran Islamic Azad University of Medical Sciences. Data were collected on demographic characteristics, clinical symptoms, and laboratory findings, including complete blood count (CBC) and platelet counts. Statistical analysis was performed using SPSS version 26, with a significance level of P < 0.05. Results: Among the 154 patients with a mean age of 50.95 ± 14.95 years, 81 (52.6%) and 73 (47.4%) were female and male, respectively. Thrombocytopenia was identified in 10 patients (6.5%). No significant associations were observed between thrombocytopenia and gender, diabetes status, abdominal pain, nausea/vomiting, or gastric ulcer (P> 0.05). However, a significant association was found between positive RUT results and thrombocytopenia (P< 0.001). Conclusion: The study highlights a potential link between Helicobacter pylori infection and thrombocytopenia, as indicated by the significant association with positive RUT results. While common symptoms and comorbidities were not predictive of thrombocytopenia, active infection may influence platelet counts. Further research is needed to elucidate the mechanisms underlying this association.
