fa فرمولاسیون قرص آهسته رهش دوکسیلامین- پیریدوکسین: راهی برای رهایی از تهوع و استفراغ دوران بارداری Sustained release formulation of doxylamine-pyridoxine combination: a promising way to overcome nausea and vomiting of pregnancy داروسازي Pharmacology تجربي Experimental سابقه و هدف: تهوع و استفراغ بارداری (NVP) یکی از شایع ترین عوارضی است که تا 80 درصد از مادران آینده را درگیر می کند. یک کارآزمایی بالینی تصادفی‌سازی‌شده فاز III، اثربخشی ترکیب داکسیلامین-پیریدوکسین [تحت نام تجاری Diclectin® در کانادا] را نشان داد. ترکیبی از دوکسیلامین سوکسینات-پیریدوکسین هیدروکلراید  با رهش کنترل شده به عنوان یک فرمول بی خطر و قابل تحمل برای زنان باردار در نظر گرفته شده است. با توجه به شیوع بالای NVP در زنان باردار ایرانی، هدف اصلی در این مطالعه فرموله کردن این محصول بود. روش بررسی: برای تهیه این فرمولاسیون از روش پرس مستقیم استفاده شد. ابتدا قرصهای حاوی دو ماده موثره تهیه شد. Eudragit L100 و L100-55 برای پوشش قرصها استفاده شد. سپس قرصهای پوشش داده شده در شرایط اسیدی قرار گرفتند تا از پایداری آنها در محیط معده اطمینان حاصل شود. تمام ارزیابیهای آزمایشگاهی انجام شد. تجزیه و تحلیل مواد موثره با استفاده از کروماتوگرافی مایع با کارایی بالا همراه با دتکتور  UV انجام شد، روش HPLC توسعه یافته و به طور کامل معتبر شد.   یافتهها: کلیه آزمونهای استاندارد قرصهای آهسته رهش که برای فرمولاسیون نهایی انجام شد نتایج قابل قبول داشتند. نتیجهگیری: با بررسی فرمولاسیونها مختلف مشخص شد که فرمولاسیونی بیشترین شباهت را به قرص برند دارد که حاوی کراس کارملوز در هسته و اودراژیت L100-55 در روکش است. بدین ترتیب فرمولاسیونی با بیشترین شباهت از نظر ویژگیها، اما با اجزای تشکیل دهنده متفاوت با برند اصلی تهیه شد.   Background: Nausea and vomiting of pregnancy (NVP) are one of the most common complications affects up to 80% of future mothers. A phase III randomized clinical trial demonstrated the efficacy of the doxylamine-pyridoxine combination [under the trade names of Diclectin® in Canada]. Doxylamine succinate-pyridoxine hydrochloride sustained release combination has been considered as a safe and well-tolerated formulation by pregnant women. Due to the high prevalence of NVP in Iranian pregnant women, we aimed in this study to formulate this combination. Materials and methods: Direct compression method was used to prepare this formulation. First, tablets containing two active agents were prepared. Eudragit L100 and L10055 were used to coat the tablets. Then the coated tablets, were exposed to acidic conditions to ensure their stability in the stomach environment. All in-vitro evaluations were done. The analysis of the APIs was done using high-performance liquid chromatography coupled with ultraviolet detection, the HPLC method was developed and fully validated. Results: Based on the dissolution test and release profile, the formulation containing 3.5% croscarmellose in the core and Eudragit L100-55 as coating polymer showed optimum similarity to the famous brand. In the optimized formulation, no significant release was detected in pH equal to stomach and total release happened in the intestine pH, similarity and difference factors for pyridoxine and doxylamine were 78% and 3% ,71% and 4%, respectively.
