fa تعامل هورمون انسولین با انتقال دهنده های عصبی در پاتوژنز بیماری آلزایمر Interaction of insulin hormone with neurotransmitters in the pathogenesis of Alzheimer's disease بيوشيمي باليني Cilinical Biochemistry مروري Review سابقه و هدف: ارتباط بین مقاومت به انسولین و بیماری آلزایمر (AD) به عنوان فرضیه «دیابت نوع ۳» مطرح است. این مرور نظام‌مند نقش اختلال سیگنالینگ انسولین مغزی در پاتوژنز AD و چشم‌انداز درمانی آن را بررسی می‌کند. روش بررسی: با پیروی از دستورالعمل PRISMA، جستجوی جامعی در پایگاه‌های PubMed/MEDLINE، Scopus و Web of Science  تا ۲۰۲۵ انجام شد. پس از غربالگری، 32 مطالعه وارد مرور شدند.  یافتهها: مقاومت به انسولین مغز یک عامل پاتولوژیک کلیدی در AD است که از طریق مکانیسم‌های مستقلی عمل می‌کند. این حالت با کاهش فعالیت آنزیم تخریب‌کننده انسولین (IDE)، پاکسازی آمیلوئید-بتا (Aβ) را مختل کرده و با اختلال در مسیر PI3K/Akt، منجر به فعال‌سازی GSK-3β  و هیپرفسفوریلاسیون تاو می‌شود. این فرآیندها همراه با اختلال در انتقال عصبی و التهاب، چرخه معیوب تخریب عصبی را شکل می‌دهند و از مفهوم دیابت نوع ۳ حمایت می‌کنند. در مقابل، درمان‌های نوظهور مانند انسولین داخل بینی و آگونیست‌های GLP-1 در مطالعات اولیه، پتانسیل بهبود نشانگرهای مولکولی و شناخت را نشان داده‌اند. نتیجهگیری: مقاومت به انسولین مغزی یک جزء علیتی مهم و قابل هدف‌گیری در پاتوژنز AD است. هدف‌گیری این مسیر، یک راهبرد درمانی امیدبخش محسوب می‌شود. با این حال، چالش‌هایی مانند استانداردسازی اندازه‌گیری، طراحی کارآزمایی‌های دقیق و عبور از سد خونی-مغزی پیش از کاربرد بالینی گسترده نیازمند توجه است. پژوهش‌های آینده باید بر شناسایی بیومارکرها و توسعه درمان‌های ترکیبی متمرکز شوند.   Background: The association between insulin resistance and Alzheimer's disease (AD) has been proposed as the "type 3 diabetes" hypothesis. This systematic review investigates the role of impaired cerebral insulin signaling in the pathogenesis of AD and its therapeutic prospects. Materials and methods: Following the PRISMA guidelines, a comprehensive search was conducted in PubMed/MEDLINE, Scopus, and Web of Science databases up to 2025. After screening, 32 studies were included in the review. Results: Cerebral insulin resistance is a key pathological factor in AD that acts through independent mechanisms. It impairs amyloid-beta (Aβ) clearance by reducing the activity of insulin-degrading enzyme (IDE) and by disrupting the PI3K/Akt pathway, leads to GSK-3β activation and tau hyperphosphorylation. These processes, together with impaired neurotransmission and inflammation, form a vicious cycle of neurodegeneration and support the concept of type 3 diabetes. In contrast, emerging therapies such as intranasal insulin and GLP-1 agonists have shown potential to improve molecular markers and cognition in early studies. Conclusion: Cerebral insulin resistance is an important and targetable causal component in the pathogenesis of AD. Targeting this pathway is a promising therapeutic strategy. However, challenges such as standardization of measurement, rigorous trial design, and crossing the blood-brain barrier require attention before widespread clinical application.
