The positive predictive value (PPV) of cardiac nuclear scans for diagnosing coronary artery stenosis and comparison with angiography findings

Background: Coronary artery disease is the most common known cause of mortality and disability among men and women in developing and industrialized countries. The aim of this study was to investigate the agreement between cardiac scan and coronary angiography of patients referred to the Nuclear Medicine Department of Tehran Heart Hospital, which was performed in 2024.
Materials and methods: This cross-sectional study was conducted using a random sampling method on 114 patients with positive scan results who referred to the angiography department of Tehran Heart Hospital. Data related to coronary artery disease risk factors were collected using a checklist. Statistical analysis was performed using analytical test and equality of correspondence ratio using SPSS27 software.
Results: Overall agreement percentage, regardless of gender, was 58.77%. The agreement rate of heart scan with coronary angiography of patients with underlying risk factors included: having a history of MI (90.9%), history of angioplasty (87.5%), genetic history (81.08%), positive exercise test (68.96%), hypercholesterolemia (65.57%), typical cardiac pain (59.01%), anemia (56.66%), diabetes (53.7%), high blood pressure (45.58%), and renal failure (41.17%).
Conclusion: Combining heart scanning with coronary angiography of patients based on the results of the scan can help increasing diagnostic accuracy and can also predict the risk of future cardiac events. In this way, waste of time and additional costs can be avoided.
